In the Third Supplement to The London Gazette of Friday, the 29th of January, 1946.

The citation reads:

For gallantry in the face of overwhelming odds whilst serving in H.M.S. Encounter during her last action in the Java Sea on 1st March, 1942:
Distinguished Service Medal. Chief Petty Officer Alfred John ADCOCK, C/J. 72656.
